Change of Ownership and/or Location

Licenses are not transferable to new owners or to a new location. The Board of Pharmacy must approve all changes in ownership and/or location BEFORE the change occurs. A new license number will be issued.

The following shall constitute a change of ownership and requires a new license application. Any transfer of a beneficial interest in a business entity licensed by the board, in a single transaction or in a series of transactions, to any person or entity, which transfer results in the transferee's holding 50% or more of the beneficial interest in that license.

You must complete and submit a new application and follow the instructions within the application for a change of ownership and/or location. Please submit an application at a minimum of 30 DAYS PRIOR to the changes occurring.
