Available Information Regarding Licensees

The following information regarding the license status and official action taken in connection with a licensee, if known, shall be disclosed to members of the public upon request.

Licensing Information:
  • Licensee Name
  • License Number
  • Name of Licensed Facility Owner (including the corporation name and corporate officers) and the Pharmacist-in-Charge
  • Address of Record
  • Date Original License Issued
  • License Expiration Date
  • Current License Status
Administrative Information and Actions:
  • Letter of Admonishment
  • Citation
Discipline Information and Actions:
  • Referral for formal Disciplinary Action
  • Accusation/Petition to Revoke Probation
  • Board Decision
  • Temporary Restraining Order, Automatic Suspension Order, Summary Suspension Order or Interim Suspension Order
  • Penal Code 23 license restrictions
