Public Disclosure of Board Disciplinary Records
The law provides that the following information is public and will appear on a record if applicable to a licensee:
- If a licensee has been disciplined or formally accused of wrongdoing by the board.
- If a licensee is currently on suspension and/or probation with the board.
- If a license has been revoked or voluntarily surrendered.
Current website information on Board of Pharmacy disciplinary actions only goes as far back as January 2005 following the effective date of the disciplinary penalty (e.g., revocation, surrender, suspension, probation, etc.)
Disciplinary actions rendered by the Board of Pharmacy and penalties imposed become operative on the effective date of the action except in situations where the licensee obtains a court-ordered stay through the appeal process. This may occur after the publication of the information on this website. For updated information on stay orders and appeals, please contact the board at the address below.
