Health Services Registry – Notify the Board of Changes or Discontinuance

If you are changing or discontinuing health services, or if you are changing your address, submit the information below to the California State Board of Pharmacy.

Check services you are providing, including current and new services. If you are no longer providing any services, check “No services.”
